Wegovy Pill: Injection-Level Weight Loss in a Tablet
FDA approved the Wegovy pill on Dec 22, 2025 and it reached pharmacies in January 2026. In OASIS 4, patients on 25 mg daily lost a mean 13.6% of body weight, in the range of injectable Wegovy 2.4 mg. By mid-2026 it had passed 5 million US prescriptions.
For patients who prefer a pill over an injection, it is a real option rather than a compromise. NovoCare self-pay is $149/mo at the 1.5 mg starting dose, $199 at 4 mg and $299 at 9 and 25 mg; Ro and Hims/Hers charge the same plus membership. On Medicare the GLP-1 Bridge covers it at $50/mo through Dec 31, 2027. Detail on our Wegovy pill page and in the GLP-1 cost guide.
Foundayo (Orforglipron): Lilly's Non-Peptide Pill
Foundayo was approved Apr 1, 2026 for chronic weight management and started shipping through LillyDirect on Apr 6. It is a small-molecule GLP-1 agonist, not a peptide, so it is absorbed without the empty-stomach ritual that oral semaglutide needs. ATTAIN-1 showed 11.1% weight loss at the top dose, less than the Wegovy pill but with simpler dosing.
LillyDirect self-pay starts at $149/mo (0.8 mg), with $199 at 2.5 mg and $299 at 5.5 to 17.2 mg (the two highest doses are $349 without a refill within 45 days). The commercial savings card is $25, and it is in the Medicare GLP-1 Bridge at $50/mo. A type 2 diabetes indication is under FDA review after the ACHIEVE-4 cardiovascular safety readout (Sep 30, 2026).
Ozempic Tablets: The Diabetes Pill
Rybelsus, the first oral GLP-1 (2019), is FDA listed as to be discontinued June 4, 2026 and replaced by Ozempic tablets, approved Feb 4, 2026 for type 2 diabetes at 1.5, 4 and 9 mg daily. The molecule is the same oral semaglutide as the Wegovy pill, at diabetes doses. NovoCare self-pay is $149, $199 and $299 per 30 tablets; the savings card brings commercially insured patients to as little as $25/mo. For weight loss, the Wegovy pill is the on-label choice; for diabetes, Ozempic tablets.
How to Take Oral Semaglutide (Critical Protocol)
Oral semaglutide absorption is poor and highly sensitive to what is in your stomach. This applies to the Wegovy pill and Ozempic tablets, not to Foundayo. To get the dose your clinician intends, follow this protocol every single day:
- First thing in the morning. Before anything else: no coffee, water, or food yet.
- Take the tablet with no more than 4 oz of plain water. Not coffee, not juice, not seltzer. Plain water only.
- Wait at least 30 minutes before any food, drink, or other oral medication. 45 to 60 minutes is better.
- Do not split, crush, or chew the tablet. The SNAC absorption-enhancer mechanism requires the intact tablet.
Skipping the protocol, even a sip of coffee within the 30-minute window, can cut absorption by half or more. This is the single biggest reason patients do not get expected results.
Side Effects to Expect
Same profile as injectable GLP-1s, concentrated during titration:
- Nausea (most common)
- Diarrhea or constipation
- Vomiting
- Decreased appetite, fatigue, headache
- Rare risks: pancreatitis, gallbladder disease, acute kidney injury from dehydration (added to the Wegovy label Aug 2025), severe GI reactions (Oct 2025)
FDA boxed warning for medullary thyroid carcinoma based on rodent studies: do not use if you or a close relative have a history of MTC or MEN2 syndrome. In January 2026 FDA asked for the suicidal-ideation warning to be removed from the Wegovy, Zepbound and Saxenda labels after a review of 91 trials found no increased risk.
What's Coming Next in Oral GLP-1
- Foundayo for type 2 diabetes (Eli Lilly): submitted to FDA in Q2 2026; no decision date disclosed.
- Ozempic 25 mg tablet for diabetes (Novo Nordisk): FDA decision expected around end of 2026 (secondary reports).
- CagriSema (Novo Nordisk, injectable): FDA decision expected Q4 2026, not a pill.
- Retatrutide (Eli Lilly, injectable): FDA filing planned Q1 2027; not expected in pharmacies before 2028.
- Danuglipron (Pfizer): discontinued in 2025.

How to Choose Between Pill and Injection
Choose a pill if:
- You strongly dislike injections
- You want the lowest self-pay price ($149/mo starting dose for the Wegovy pill or Foundayo)
- You can commit to the daily empty-stomach protocol (oral semaglutide), or you pick Foundayo to avoid it
- You travel often (no refrigeration required)
Choose an injection if:
- You want once-weekly dosing
- You want the highest efficacy (Zepbound still leads at about 20%)
- You will not reliably follow the daily empty-stomach protocol
