01 Price by dose

How Much Is Zepbound Without Insurance? Price by Dose

Lilly sells Zepbound for cash through LillyDirect at one price per dose, whether you get the 4-dose KwikPen or a month of single-dose vials. The lower $449 tier for 7.5 mg and up applies only if each refill is completed within 45 days of the last delivery. Miss that window and 7.5 mg costs $499 while 10 to 15 mg costs $699.

Each fill is a 1-month supply: one KwikPen with 4 weekly doses, or 4 single-dose vials. The label lists 5 mg, 10 mg and 15 mg as maintenance doses; your prescriber sets the dose. Prices as of Oct 2026.

Do vials and KwikPens cost the same?

Yes, as of Oct 2026. Lilly's February 23, 2026 release on the KwikPen launch says all doses are available "in either multi-dose KwikPen or single-dose vial at the same self-pay price," starting at $299. TrumpRx.gov lists Zepbound vials at the same $299 / $399 / $449 tiers. Before that date vials were the cheaper format, which is why older articles still quote a vial discount.

02 The math

Zepbound Cost for the First Year

The label starts everyone at 2.5 mg for 4 weeks and raises the dose by 2.5 mg no sooner than every 4 weeks. The table below assumes the fastest climb to 15 mg and 13 fills in 52 weeks.

Self-pay, refills on time

Total, 52 weeks, 13 fills $5,637 $8,187

Past 7.5 mg the dose no longer changes the price, so 10 mg and 15 mg maintenance cost the same $5,637 a year. The 45-day rule is worth $2,550 a year at maintenance doses, so a refill reminder pays for itself. At TrumpRx's $1,087 original price, 13 fills would run about $14,131.

03 LillyDirect

LillyDirect Zepbound: How the Self-Pay Offer Works

LillyDirect is Lilly's own online pharmacy and telehealth hub; it is where the $299 to $449 cash prices live. You still need a prescription from a US prescriber, either your own doctor or a telehealth clinic.

  1. 01

Get a prescription for the KwikPen or vials.

The self-pay offer covers "a 1-month prescription of the Zepbound KwikPen for single-patient use or 4 vials." It does not cover the single-dose Zepbound Pen (autoinjector).

  2. 02

    Have it sent to LillyDirect.

The prescriber sends it to LillyDirect Pharmacy, which ships it to your home.

  3. 03

    Pay by dose.

$299 at 2.5 mg, $399 at 5 mg, $449 at 7.5 mg and up with an on-time refill.

  4. 04

    Refill within 45 days.

The clock runs from the delivery or receipt date of your last fill. After 45 days the regular price applies: $499 for 7.5 mg, $699 for 10 to 15 mg.

  5. 05

Medicare patients can pay cash too.

Lilly's page notes that Medicare patients can also choose self-pay at the same prices.

LillyDirect Customer Care, 844-559-3471, 9am to 7pm ET, Monday to Friday (as listed on TrumpRx.gov, Oct 2026).

How much does Zepbound cost with insurance?

With coverage, your real cost is your plan copay minus the card, as low as $25 a month until you hit the $1,300 yearly cap. Coverage itself depends on your employer: in KFF's 2025 survey, 43% of firms with 5,000+ workers covered GLP-1s primarily for weight loss (28% in 2024).

05 Cheapest way

Cheapest Way to Get Zepbound Without Insurance

Ranked from lowest to highest monthly cost as of Oct 2026. Pick the first line that fits you.

  1. $50

    1. Medicare Part D and you qualify: $50/mo.

The CMS Medicare GLP-1 Bridge charges $50 a month for the Zepbound KwikPen (not vials) from Jul 1, 2026 to Dec 31, 2027, if you meet a BMI tier. Details: Medicare GLP-1 Bridge.

  2. $25

    2. Commercial plan that covers it: $25/mo.

Use the savings card on top of your plan, up to $1,300 a year.

  3. $299+

    3. No coverage: LillyDirect self-pay, $299 to $449.

Refill within 45 days every time; at 7.5 mg and up that saves $50 to $250 a month versus a late refill.

  4. $299+

    4. Same price, different door: TrumpRx.gov.

TrumpRx lists the same $299 / $399 / $449 tiers and sends you to LillyDirect to order; it adds no second discount. See TrumpRx GLP-1 prices.

  5. +fee

    5. Telehealth clinic: drug price plus a fee.

Useful if you need a prescriber; the clinic adds a membership on top of Lilly's price (table below).

  6. $499+

    6. Retail cash at Costco: higher.

Costco lists the KwikPen at $499.22 (2.5 to 7.5 mg) and $699.21 (10 to 15 mg).

Compounded tirzepatide is cheaper on paper (October 2026 index: $222 median, range $129 to $399 across 14 telehealth providers), but it is not FDA-approved and is not Zepbound. Since the tirzepatide shortage ended on October 2, 2024, only patient-specific 503A compounding remains lawful. More: compounded tirzepatide.
