How Wegovy Works
Wegovy contains semaglutide, a long-acting GLP-1 receptor agonist. By activating GLP-1 receptors in the pancreas, brain, and gut, it slows gastric emptying, reduces appetite, and improves glucose handling, which together produce sustained weight loss.
The molecule is the same as in Ozempic and Ozempic tablets (Rybelsus is FDA listed as to be discontinued June 4, 2026); what differs is the dose range and approved indication. Wegovy is approved for chronic weight management at higher doses than Ozempic, which is why it produces somewhat greater weight loss on average.
Who Qualifies for Wegovy
- Adults with BMI 30 or higher (obesity), or
- Adults with BMI 27 or higher with at least one weight-related condition (hypertension, type 2 diabetes, sleep apnea, dyslipidemia, cardiovascular disease, prediabetes), or
- Adolescents 12+ with obesity
Medicare GLP-1 Bridge eligibility is stricter: BMI 35 or higher; or BMI 30 or higher with heart failure with preserved ejection fraction (HFpEF), uncontrolled hypertension or chronic kidney disease stage 3a or higher; or BMI 27 or higher with prediabetes, a prior heart attack, a prior stroke or symptomatic peripheral artery disease. Check where you stand with our BMI calculator. You should not use Wegovy if you or a close relative have a history of medullary thyroid carcinoma or MEN2 syndrome.
Dosing and Titration
To minimize gastrointestinal side effects, the Wegovy pen is titrated up gradually over 16 to 20 weeks:
- Weeks 1-4: 0.25 mg weekly
- Weeks 5-8: 0.5 mg
- Weeks 9-12: 1 mg
- Weeks 13-16: 1.7 mg
- Week 17+: 2.4 mg maintenance weekly (Wegovy HD 7.2 mg is a further step for some patients)
The Wegovy pill is sold in 1.5, 4, 9 and 25 mg tablets and titrated over several months to 25 mg. Some patients titrate slower to manage side effects. Do not increase faster than recommended; doing so concentrates side effects without improving outcomes.
How Much Weight You'll Actually Lose
In STEP-1 (Wegovy 2.4 mg injection, 68 weeks):
- Average weight loss: 14.9% (compared to 2.4% on placebo)
- 69% of patients lost at least 10% of body weight
- 32% lost at least 20% of body weight
In OASIS 4 (Wegovy pill 25 mg): 13.6% mean weight loss. Individual results vary based on adherence, lifestyle, and biology.
Side Effects
Most common (during titration):
- Nausea (44%)
- Diarrhea (30%)
- Constipation (24%)
- Vomiting (24%)
- Abdominal pain, fatigue, headache, injection-site reactions
Rare but important: acute pancreatitis, gallbladder disease, severe GI reactions (added to the label Oct 2025), acute kidney injury from dehydration (Aug 2025). FDA boxed warning for medullary thyroid carcinoma, unchanged. In January 2026 FDA asked Novo to remove the suicidal-ideation warning after a review of 91 trials found no increased risk. Discontinue and seek care for severe persistent abdominal pain or signs of allergic reaction.
How to Get Wegovy
Two practical paths:
- In-person primary care, obesity medicine, or endocrinology. Best for insurance coverage and integrated metabolic workup.
- US telehealth. Ro and Hims/Hers sell Wegovy at the NovoCare prices (pill $149, pen $199 then $349) plus a membership of $39 for the first month and $149/mo after; see Ro GLP-1 reviews. PlushCare ($19.99/mo, $129 visit without insurance) and Noom Med prescribe for pharmacy fill. Not every service is available in every state.
Insurance, Medicare and the NovoCare Program
Employer coverage (KFF 2025): 43% of firms with 5,000+ workers cover GLP-1s primarily for weight loss (28% in 2024), 30% of firms with 1,000 to 4,999 workers, 16% of firms with 200 to 999. With coverage, the Wegovy savings card brings the copay to as little as $25/mo (max savings $100/mo). Details in GLP-1 cost with insurance.
Medicare: since Jul 1, 2026 the GLP-1 Bridge covers Wegovy pen and pill at a flat $50/mo copay for eligible Part D enrollees, through Dec 31, 2027. Anyone who got a GLP-1 through Part D in 2026 is excluded.
NovoCare self-pay: pen $349/mo ($199 first two fills through Dec 31, 2026; HD $399), pill $149 to $299/mo by dose. Requires a valid prescription; no insurance needed. Start at novocare.com. Novo cuts the list price to $675/mo on Jan 1, 2027; self-pay is unchanged. Full breakdown on the Wegovy cost page; updates on GLP-1 news.
Long-Term Picture
Wegovy is a long-term therapy. STEP-4 showed that patients who stopped semaglutide after a 20-week run-in regained about two-thirds of lost weight within a year. Plan a multi-year continuation strategy with your clinician.
